When you need medical care, who will speak for you if you are not able? Find out how to make sure your medical care choices are honored on Saturday, April 21 at the Ramsey County Library in Roseville.
Roseville’s Community Health Awareness Team (CHAT) is hosting a free Advance Care Planning (ACP) discussion from 10:30 a.m. to 12:30 p.m this Saturday. ACP is a process that helps you consider and record your choices for future health care decisions.
The event will include a panel discussion followed by small group discussions with ACP facilitators. There will also be an opportunity to get help creating a Health Care Directive.
